Parastomal Intestinal Evisceration

چکیده
A 23-year-old male with a history of Crohn’s disease and prior ileostomy, presented to the emergency department complaining of his “intestines coming out.” The patient reported feeling pain and a fullness in his ileostomy bag after being punched in the abdomen. The bag was removed and an evisceration of approximately two feet (60 cm) of his small intestine around the ileostomy stoma was noted (Figures 1 and 2). There was no evidence of strangulation, and approximately one foot of the intestines was reduced with slow pressure from a gloved finger. The remainder could not be reduced and was covered in warm, saline-soak gauze, and a surgical consult was obtained. The patient received a laparotomy with diversion of his ileostomy to his left side but did not require resection of the small bowl. Parastomal intestinal evisceration is a rare complication with only one case report in the literature.1 Rates of stoma complications vary across studies, but the most common complications are: skin irritation, necrosis, stenosis, prolapse, and parasternal hernia.2-4 While rare, this is an important complication that requires mandatory surgical consultation. Management strategies include reduction and fascial repair, or laparotomy.5

Immediate postoperative parastomal end sigmoid hernia resulting in evisceration and strangulation
Parastomal evisceration is a very rare complication occurring after stoma formation. We report the case of this complication which occurred within 3 days status post end sigmoid colostomy in a 69-year-old male who initially presented with perianal infection-severe necrotizing fasciitis. This case highlights the significance of the size of a stomatal aperture and should remind general surgeons o...
متن کاملIntestinal Perforation Following Ileoscopy Through a Prolapsed Stoma in an Pediatric Intestinal Transplant Recipient With an Unrecognized Parastomal Hernia
Ileoscopy with mucosal biopsy is fundamental in the management and surveillance of inflammatory bowel disease patients and intestinal transplant recipients. There is a paucity of data describing the risks of ileoscopy in the presence of a prolapsed stoma. Parastomal hernias are frequently associated with prolapsed stomas. We report the first case of perforation during ileoscopy in the setting o...
متن کاملAcute cholecystitis in a parastomal hernia causing a small bowel obstruction
A parastomal hernia is the abnormal protrusion of intra-abdominal tissue and organs through a defect in the abdominal wall around an ostomy. Commonly, they involve intra-abdominal fat, omentum or bowel. However, there are rare cases that involve other organs. We present the case of an 89-year-old gentleman with a gallbladder in his parastomal hernia. Due to his acute cholecystitis, the distende...
متن کاملA new classification of parastomal hernias--from the experience at Bielański Hospital in Warsaw.
UNLABELLED Parastomal hernia is the most common local stoma complication. The incidence of parastomal hernia reaches 48%, and in a certain proportion of patients it is considered to be an inevitable consequence of stoma formation. The current classifications of parastomal hernias (Rubin, Devlin) are of little clinical value, which is supported by the fact that they have not been used in any pub...
متن کاملSigmoidostomy-related parastomal hernia.
BACKGROUND AND AIMS The aim was to examine prevalence and treatment of parastomal hernias in patients with sigmoidostomy. MATERIALS AND METHODS In 2009, the medical records of 447 consecutive patients operated with a sigmoidostomy from January 1999 to December 2008 were retrospectiely reviewed. 119 dead patients (26.6%) were excluded because of short follow up (n = 59) and insufficient clinic...
